We are seeking a passionate and student focused Case Manager- Tailored Learning to join our growing team on a Full-time basis, primarily located at our Payneham site. In this role, you will support students who face barriers to education by providing personalised guidance, encouragement and practical strategies that help them stay engaged and strengthen their skills across living, learning and transition pathways. Working closely with students, teachers and school employees to deliver tailored supports that build confidence, resilience and wellbeing while improving learning outcomes. Key Responsibilities: - Provide client services within SYC’s Case Management model.
- Support delivery of a learning program working collaboratively with the Tailored Learning Teacher and other Case Manager’s.
- Work to achieve outcomes specified in funding contracts and within program guidelines.
- Work collaboratively with colleagues and stakeholders to undertake appropriate entry screening for clients.
- Conduct holistic assessments of young people to establish their needs, barriers, and goals.
- Develop a case management plan with young people within funding contracts / guidelines.
- Provide direct service including therapeutic interventions to assist young people to meet their case plan.
About You As someone who enjoys working in a highly collaborative team environment, you will be passionate about supporting young people to engage in an alternative learning space. The role is multi-faceted including maintaining and developing relationships with schools, other relevant stakeholders, and participants to promote and achieve success on their educational journey! The successful candidate will demonstrate: - Minimum of one or more of the following qualifications is essential: Certificate IV or Diploma in Youth Work or Youth Justice, or Bachelor of Social Work or Psychology, or Certificate IV in Child, Youth and Family Intervention (Child Protection).
- Experience in engaging and working with young people who are disadvantaged by homelessness, an offender history, family breakdown, poverty or lack of education.
- Previous experience in conducting client assessments, developing case plans and writing case notes and reports.
- Demonstrated experience in delivering group programs.
Key screening and requirements: - Be willing to complete a Working with Children Check and National Police Check.
- Be eligible to work in Australia
- Have possession of a current valid Driver’s License, willingness to drive, and your own registered vehicle with a 4–5-star ANCAP safety rating.
- Hold and maintain a current RRHAN-EC certificate and Safe Environments Training certificate.
- Willing to undertake Mental Health First Aid and Disability Awareness training.
